These are the first appointment intake forms that we ask our patients to fill out. You can fill the PDF documents out at home and fax them or email them to our office.

Normally we ask patients to come in 30 minutes prior to your appointment time in order to fill out paperwork and have X-rays taken. If you get these documents to us prior to your arrival, you can save some time during your first appointment.
